ALL RED CARD AND INCIDENT REPORTS MUST BE COMPLETED BY A REASONABLE HOUR THE DAY OF GAME!

Please see all below that is necessary for filling out a complete report... incomplete reports are not acceptable:

  • Gender

  • Level

  • Date of Game

  • Start time of Game

  • Time of Red Card or Incident

  • How long the Game was Delayed

  • Referee Name, Email, and Phone Number

  • Partner(s) Name, Email, and Phone Number

  • Site of the Game / Location of the Incident

  • Home and Away Team Information

  • Score of the Game at the time of the Red Card / Incident

  • Final Score of the Game

  • Offending Individual(s) Name(s) and Jersey Number(s)

  • Who the action was against (opponent, teammate, spectator, official, etc.)

  • Two Yellow Cards or Straight Red Card

  • Specific NFHS Rule that was violated / broken in order to receive a disqualification (ejection / red card)

  • Was the individual warned in any way prior to being disqualified, includes verbal warnings and first yellow cards

OTHER IMPORTANT ITEMS ABOUT RED CARDS / INCIDENTS:

  • This information is all going to be in arbiter, on the game roster, in the rule book, and/or on or in your own game notes... there is no excuse for not being able to complete a report

  • The official who actually shows the red card(s) is responsible for filling out and or filing the report

  • Work with your partner after the game to make sure all information is correct before submitting a report

  • In order for any card to count, including to players, coaches, and/or other adults listed on the roster, you must show the card

  • When describing a red card for serious foul play... DO NOT USE THE WORDS "reckless" and/or "careless"

  • A red card for serious foul play... YOU MUST USE "endangers the safety of the opponent" and/or "excessive force"

  • Serious Foul Play CANNOT occur if the ball is out play... if the ball is out of play or not being played, that is VIOLENT CONDUCT

  • Language that is punished with a red card MUST BE QUOTED EXACTLY in the report as it was said/heard

  • DO NOT ANSWER ANY QUESTIONS from a school, coach, liaison, parent, player, etc., after you have issued a red card or an incident has occurred - if you are contacted, by anyone above, do not respond and call Daniel, Victor, and/or Carlos immediately!
